You can make it any size with any margin guides you want, then I just duplicate the page as I go. So, the set up is only one time really. Before you get too far, I would do one or two sample layouts, export it as a jpeg and then upload the file to MyPublisher.
Then drag and drop to make sure that everything looks they way you want it to in the preview. That way you can make adjustments before you lay out the rest of the book. You can either export it as a spread or individual pages. Typically if I have a two page spread where an element crosses the middle, I usually export the two pages as a whole. I really like being able to keep and have full control over my designs so I can print them anywhere. Hope that helps!

As for changing sizes, you can resize your canvas in Fotofusion. You can also select all of the elements on a page and then move your mouse to reduce the size of everything at once. I tried it a tiny bit several years ago but never got hooked. My current workflow stinks which is why I might try to look into FotoFusion. That was great but the process took way too long too much editing in PSE to incorporate shading, etc.
However, I have the final. The ability to reprint from originals years down the road is huge for me—the whole concept of having only a hard copy of all the hard work in creating a digital photobook seems ludicrous to me in this day and age.
